This is a downloadable software template designed for Systm One users. It serves to fulfil the NHS England national evaluation program for the First contact practitioner data collection service.

It is a working version that is in current use.

• Red flag screening tick box checklist (evidence based)
• National evaluation program codes (compliance)
• Useful links to URLs for info leaflets, societies, governing bodies

With all the read codes in tick box format. As such it resolves problems with the previous template issued by CSP, which only allows single item selection in the Referrals or Investigations subsections.
